Thanks again @Humblefish for your help with my Pop Eye Wrasse.

To bring everyone else up to speed. I received a Golden Rhombroid Wrasse from a non-local lfs. The fish had been delivered with a Pop Eye and presumably was injured during shipping.

Upon arrival, the lfs put the fish through a round of prazi and then it was placed in their main system which runs with a low level of copper.

The fish remained in the lfs for a month. No one wanted a fish with pop eye (so sad!).

Fast forward to now...I reached out to @Humblefish for advice on how to treat this fish. The fish is eating, shows no other signs of injury, disease, or trauma. The pop eye itself was clear and showed no signs of cloudiness.

Per the advice I received, I have been treating the fish for about 7 days now. He is in a 10gl IM10 tank. All filter media, aside from filter floss has been removed.

Day 1 - Added 2 tbs of Epson Salt to help ease the swelling. Began first package of Erythromycin. Began to drop the temperature from 78 to a recommended temperature of 74-75. Also began to lower salinity from 1.026 to 1.017. The purpose of these last 2 steps is to help reduce the metabolic demands on the fish and slow the eye infection.

Day 2 - Added next dose of Erythromycin. Dropped temp to 76.5 and dropped salinity to 1.023

Day 3 - 25% water change. Dosed 2 tbs of Epsom Salt. Lowered tank temp to 75. Salinity now sits at 1.020

Day 4 - Dosed Erythromycin. Final temp drop to 74. Fish is doing good, eating well. The eye doesnt appear as swollen.

Day 5 - Temp level maintained. Salinity drop to 1.017. Dosed Erythromycin.

Day 6 - Temp/Salinity maintained. 25% waterchange. Dosed 2tbs of Epsom Salt and dosed Erythromycin.

Day 7 - Erythromycin dosed. Temp/Salinity Maintained.

You may have noticed that I didn't do a sudden drop in temp or Salinity. Here is my thinking, Wrasse are sensitive to Copper to begin with (and in my opinion should not be treated with cooper unless it is warranted and closely monitored), and this wrasse sat in copper for a month. So not only was he acclimating to a copper free environment, he was also going through temp and salinity changes. So to minimize any unneccessary perceived stress, I took the slow approach and so far it is paying off.

This has been an interesting journey thus far and I have learned quite a bit from Bobby! The fish has one more round of Erythromycin left and will then run him through some Minocycline. I would have actually run the Minocycline and Erythromycin at the same time, but had to wait on shipping.

Just to explain the rationale behind some of the treatments used:

Epsom Salt - Helps to relieve swelling which is what "Popeye" indicates (eye trauma). Some will point out that Epsom Salt is just magnesium sulfate, which is already present in seawater. However, there is strong anecdotal evidence (possibly an impurity found in the Epsom Salt?) that this treatment helps with Popeye.

Erythromycin and Minocycline - These are two antibiotics which are oftentimes useful for treating Streptococcus eye infections in fish. The most common symptom is white or haziness over the retina. However, a cloudy eye can also be flukes (monogeneans) inside the eye.

Lower temp & SG - Lowering both helps to slow everything down. Which reduces the metabolic demands on the fish, reduces stress, and decreases damage from a fish's immune response. I recommend 74F and 1.017 SG in most cases when you are dealing with a sick fish.

Checked on Mr. Wrasse today in the light and while I couldn't get him to swim still for a picture I will say that the swelling has decreased by 50-60% and I have a clear view of the pupil, no visible clouding or haziness.

Last dose of Erythromycin will be tomorrow. At that time I will start the other antibiotic if warranted, If not I will likely keep him in QT and continue dosing the Epsom Salt until the swelling is 100% resolved unless @Humblefish has another recommendation [emoji846]

The original recommendation to me was to do BOTH. The only reason I didn't is because I didn't have the other antibiotic mentioned onhand and I didn't want to wait to start treatment.

Good luck with Mr. Wrasse. I will order antibiotics Today so I can treat one of mine. I think he got his eye injured and has a bit of PopEye with some cloudiness.
I did a FW dip and Methylene Blue and Rally Ruby baths, and no flukes.

Interesting. If I Google API E.M. Erythromycin, I see API E.M API E.M. Erythromycin. If I go to Amazon, that product doesn't show at all, but shows Furan-2 instead.
Which one where you able to find?

I had the API Erythromycin but had to get the minocyclin through an online vet med site, I quoted it a few posts up.

In most cases, Erythromycin alone will work because most eye infections are caused by gram-positive bacteria.

However, Minocycline treats gram-negative eye infections.

The beautiful thing is Erythromycin and Minocycline work very well together, so there are no downsides to combining them (other than the cost).

Keep treating until the issue is 100% resolved. If you stop antibiotics prematurely, the bacteria can make a comeback.

Keep treating until the issue is 100% resolved. If you stop antibiotics prematurely, the bacteria can make a comeback.

@TaylorMade treatment for your clown with popeye should be similar

@sneezebeetle have an updated pic of the wrasse?
Still doing well. I think the reduced temp and SG have definately slowed the fish down over all, will try and get a pic today if I can persuade him to come out of his safe place.
