Does this look like tail fin rot or just fryed fins? He does spend a lot of time in a cave with lots of things that can poke and scratch him. If fin rot, how do I treat it?

At first royal grammas will hide a lot so it’s normal if it doesn’t come out a lot. As far as the fins it looks like fin rot but I haven’t dealt with that issue in ages and when I did notice it on my fish it was to late as it died the next day. I’d say just feed it good 2 times a day with mysis.

It is fin rot. I would treat with Maracyn Two for Saltwater Fishes. Have you tested water quality?

Other fish seem to be fine. I also have 2 clowns and 2 neon gobies. Emerald crab, pompom crab, 2 hermits, 5 snails, several coral. Tank setup for 2.5 mo.

I’ve had him for about 3 weeks. Noticed the fins about a week ago, it’s gotten a little worse since then.

It's also possible the clowns are picking on him. Clowns are known for attacking as a fish is swimming away, so it may be possible they are nipping his fins as he swims away if they are defending their area or nem. You might try treating him for fin rot just in case but also be aware of the possibility of the former.

I'd dose Stress Guard once, as that contains an antiseptic (reef safe) for treating minor issues like this. You can also food soak vitamins/Beta-glucan to help boost his immune system.
