Can someone please help me to identify the white spots on my fish and provide advice on what to do.

Might be some flukes. Have you tried a H2O2 dip yet? He looks like he could use one. LT eye also looks bad. Hard to say what that is in-between the eyes on the top of the head. You’d need to get a better pic.
