ok so i had a yellow eye tang and well it freaked out then died? all of my levels read good and all of my other fish are fine? anyone have any ideas of why this happend?

i had it for about three days....i have two damsels and its a 56 gallon tank with 30 ibs of live rock....also a BTA well the damsels never gave it a problem because they were scared of it....when i said freaked out i mean that it just swam all over the tank really fast then it hit the sand and just layed on its side breathing heavy then stoped and turned black....i let it acclimate for about 45 mins..

How long has the tank been set-up previous to the addition of the tang?? This is also important.
